Complete all required paperwork - There are several forms to review and complete. Some of these are legal forms and others give us helpful background on your child. *All forms must be completed by a parent or legal guardian.* - **Log in to your account and navigate to the assignments page to find:** - **Consent form** - **Therapy disclosure statement/assent form** - **NOPP** - **Multiple caregiver form** - **New Patient Questionnaire** - **PSC-17**: a standardized questionnaire/screener from the parent/caregiver’s perspective - **Financial responsibility form** - **Release of information**: optional but important; this release helps us partner more easily with your child’s primary care provider or any other current provider (it also allows vital communication with teachers and other caregivers to happen) ![Wrapper for 4js71WjT8e3IlTX35Aib3s](https://images.ctfassets.net/96gdpqkm7elu/4js71WjT8e3IlTX35Aib3s/0ff027338855e148a4392517a4f01fac/Complete_Paperwork.svg) ## 3. Prepare your child for their appointment - You know your child best. We recommend being honest with your child and telling them that they have an appointment with us. - You can share that the first visit will be a conversation. We’ll simply ask questions to get to know them — what they like to do, what things they find more difficult, whether or not they feel like they could use some help in any areas. - The appointment is not a confrontation — your child hasn’t done anything wrong (this isn’t punishment). In fact, we hope that your child leaves that first session feeling heard and understood (even if their concern is that they’d rather just be at home). - If it will help comfort your child, please feel free to let them bring a supportive toy or object into the session. They can even introduce it to their provider. No matter what your child is here for, you (as their parent) will be closely involved in their care. The level of involvement depends on a few things: their age and stage, the presenting concerns, and what research tells us works best. Sometimes, kids meet with their provider and a caregiver checks in for a few minutes. Other times, the caregiver is the one in the session for a longer time.
