# Just a few forms, then we get to work

Starting care means a few forms to fill out. We'll guide you through all of it, and most of it can be done right from your Brightline account.

## What you'll need to complete

*Matching starts with what your child actually needs. Our team looks at:*

- Guardian verification and consent, including authorization from any additional legal guardians if applicable
- Consent for therapy and, if psychiatric services are part of care, medication management
- Financial responsibility and insurance information
- A short check-in on how you're doing as a parent, to help inform your child's care
- Paperwork tied to your child's diagnostic evaluation and feedback session
- Additional forms if needed, such as custody documentation or medication history
- Once care begins, you may also be asked to complete short digital exercises or questionnaires to help track your child's progress.
