No Surprises Act

Good Faith Estimate. If you are uninsured or choose not to use your insurance (self-pay), you have the right to a good faith estimate of expected charges when you schedule care or on request. We provide it in writing, and in an accessible format if you ask. An estimate is not a contract or a guarantee — actual charges may vary based on the care you receive. If you are billed $400 or more above your estimate, you may dispute the bill through the federal patient-provider dispute resolution process, generally within 120 days of the date on the bill. For more information, call 1-800-985-3059 or visit cms.gov/nosurprises.
Insurance and Cost Estimates. The health plans Brightline participates in as well as our self-pay pricing are listed at https://www.brightline.com/pricing/insurance. Brightline provides care through telehealth and our clinicians are not affiliated with a hospital. You may request a written estimate of expected charges before receiving care by calling (855) 278-5444. Estimates are based on what is known at the time and are not a guarantee of final charges.
Protections Against Surprise Bills. Federal and state law protect you from balance billing for emergency care and for certain out-of-network services at in-network facilities such as hospitals and surgical centers, where you owe no more than your in-network cost sharing. Brightline provides care through telehealth and does not furnish services at those facilities, so these protections generally do not apply to Brightline's services. Additional protections may apply in the state where you receive care, including protections relating to referrals to non-participating providers. If you believe you have been billed in violation of these protections, contact us at (855) 278-5444, the federal No Surprises Help Desk at 1-800-985-3059 or cms.gov/nosurprises, or your state insurance regulator, which you can find at naic.org.
