What you'll need to complete
Matching starts with what your child actually needs. Our team looks at:
Guardian verification and consent, including authorization from any additional legal guardians if applicable
Consent for therapy and, if psychiatric services are part of care, medication management
Financial responsibility and insurance information
A short check-in on how you're doing as a parent, to help inform your child's care
Paperwork tied to your child's diagnostic evaluation and feedback session
Additional forms if needed, such as custody documentation or medication history
Once care begins, you may also be asked to complete short digital exercises or questionnaires to help track your child's progress.
